Player Contract and Code of Conduct Form To be held by the parish coordinator for each player on a team. When asked by the CYO Committee to provide this contract for each player on a team, please do so as instructed.
Coaches Code of Conduct To be completed by November 13 by each head coach and assistant coach who is most likely to substitute for the head coach. This form is maintained by the parish coordinator for all coaches in the parish/school program. When asked by the CYO Committee to present this form, in the case of a random audit, please do so as instructed.
